Clinical Characteristics of Pantaloon Hernias

Abstract
Purpose: The use of laparoscopy for treating inguinal hernias has been increasing. Pantaloon hernias are uncommon and present as discrete direct and indirect hernias on either side of the epigastric vessel. This study aimed to investigate the clinical characteristics of pantaloon hernias observed during laparoscopic herniorrhaphy in a specialized hernia center.

Methods: From October 2014 to December 2016, 985 adult patients underwent inguinal hernia repair, 18 of whom had pantaloon hernias. We analyzed the clinical data of these 18 patients.

Results: All patients were males, with a median age of 59.0 (range, 34¡­74) years. Pantaloon hernias most commonly occurred on the right side (nine patients, 50%). Twelve (66.7%) patients had combined hernias in the contralateral groin. Direct hernias were the most common combined hernias (six patients, 33.3%). The median operation time, median hospital stay, and median postoperative follow-up period were 31 (20¡­40) min, 4.33 (2¡­8.1) h, and 12.5 (2¡­28) months, respectively. No major complication was noted.

Conclusion: If a pantaloon hernia is observed during surgery, the possibility of a combined hernia in the contralateral groin should be considered. Laparoscopic herniorrhaphy allows the accurate diagnosis of the type of hernia and the simultaneous treatment of all combined hernias detected. Therefore, we believe that laparoscopic surgery can be a good option for treating pantaloon hernias.
